Central Post-Acute Resource Center (PARC) Specialist – Columbia
New York-Presbyterian has a long tradition of bringing hope, health, and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities and is strengthening those that exist. Our programs build upon and tie together the Hospital’s inpatient care coordination and social work teams, post-acute care providers, and other community resources. Now, we're seeking a Central Post-Acute Resource Center (PARC) Specialist to help drive this work forward.
The centralized Post-Acute Resource Center (Central PARC) Specialist will support New York Presbyterian (NYP)’s post-acute care strategy. Under the direction of the Central PARC Program Manager, the specialist will work collaboratively with the interdisciplinary team to strengthen care transitions across the continuum and develop strong relationships with post-acute care providers. As a key member of the team, the Central PARC Specialist will play an active role in identifying and implementing strategies to enhance the post-discharge experience and outcomes for patients. This includes fostering and maintaining effective partnerships with post-acute providers, ensuring clear and consistent communication among all key stakeholders and enabling the care teams to facilitate efficient discharges. Responsibilities also include conducting follow-up calls with patients and performing periodic site visits to post-acute care facilities to identify shared opportunities for improvement and support seamless care transitions.
This is Monday-Friday position with weekends and holidays on rotation.
Preferred Criteria
Master’s Degree
Clinical background (RN, LMSW, LCSW)
Experience working with post-acute care providers (including discharge planning, care coordination and knowledge of the continuum of care planning process)
Required Criteria
Minimum of 5 years in healthcare environment
Bachelor’s Degree
Basic knowledge of performance improvement concepts and EMR documentation

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
